You are required to submit a signed notice of the name change to the Attorney General within thirty (30) days of the name change. The notice should state your commission and expiration dates, your former name and new name, and the effective date of your new name. Additionally, you must provide a certified copy of a document evidencing your name change, such as a marriage certificate or court order, and notify the insurance company issuing your notary bond of your name change. Upon receipt of the signed notice, the Attorney General will schedule an appointment for you to sign a specimen card. See Section 33551 of the Law.
