A consumer complaint form should be filled out and can be obtained from the front desk of the Office of the Attorney General. The office may also fax the form, mail the form or e-mail the form. The online form is in PDF form so the consumer only has to download the form and fill it out. The completed form may be turned in to the office by turning it in to the front desk, mailing the form to the office address. All consumer complaints must be signed by the consumer.
